Grapefruit and medication intake: be careful!

Grapefruit is known to be an interesting source of vitamin C. Studies have also shown its benefits on diabetes and cardiovascular problems. However, be careful when eating grapefruit if you're taking medication. Hereโ€™s whyโ€ฆ

Even though it has ๐š๐ง๐ญ๐ข๐จ๐ฑ๐ข๐๐š๐ง๐ญ ๐š๐ง๐ ๐š๐ง๐ญ๐ข-๐ข๐ง๐Ÿ๐ฅ๐š๐ฆ๐ฆ๐š๐ญ๐จ๐ซ๐ฒ ๐ฉ๐ซ๐จ๐ฉ๐ž๐ซ๐ญ๐ข๐ž๐ฌ due to its composition, grapefruit is known to be involved in ๐ง๐ฎ๐ฆ๐ž๐ซ๐จ๐ฎ๐ฌ ๐๐ซ๐ฎ๐  ๐ข๐ง๐ญ๐ž๐ซ๐š๐œ๐ญ๐ข๐จ๐ง๐ฌ. A study published in 2013 listed 58 oral medications that interacted with grapefruit, 43 of which may cause ๐ฌ๐ž๐ซ๐ข๐จ๐ฎ๐ฌ ๐œ๐จ๐ฆ๐ฉ๐ฅ๐ข๐œ๐š๐ญ๐ข๐จ๐ง๐ฌ. But what is the mechanism behind these interactions?

This phenomenon is due to the ๐ข๐ง๐ก๐ข๐›๐ข๐ญ๐ข๐จ๐ง ๐จ๐Ÿ ๐๐ซ๐ฎ๐ -๐ฆ๐ž๐ญ๐š๐›๐จ๐ฅ๐ข๐ณ๐ข๐ง๐  ๐ž๐ง๐ณ๐ฒ๐ฆ๐ž๐ฌ in the intestine and liver, mainly the CYP3A4 enzyme (cytochrome P450), by furanocoumarins. When these ๐ž๐ง๐ณ๐ฒ๐ฆ๐ž๐ฌ ๐š๐ซ๐ž ๐ข๐ง๐ก๐ข๐›๐ข๐ญ๐ž๐ ๐ข๐ง ๐ญ๐ก๐ž ๐ข๐ง๐ญ๐ž๐ฌ๐ญ๐ข๐ง๐ž, drug metabolism is reduced and the amount of unmetabolized drug entering the bloodstream is ๐ข๐ง๐œ๐ซ๐ž๐š๐ฌ๐ž๐, ๐ฉ๐จ๐ญ๐ž๐ง๐ญ๐ข๐š๐ฅ๐ฅ๐ฒ ๐ฅ๐ž๐š๐๐ข๐ง๐  ๐ญ๐จ ๐จ๐ฏ๐ž๐ซ๐๐จ๐ฌ๐ž. In rarer cases, grapefruit ingestion may cause ๐š ๐๐ž๐œ๐ซ๐ž๐š๐ฌ๐ž ๐ข๐ง ๐ญ๐ก๐ž ๐š๐ฆ๐จ๐ฎ๐ง๐ญ ๐จ๐Ÿ ๐๐ซ๐ฎ๐  ๐ข๐ง ๐ญ๐ก๐ž ๐›๐ฅ๐จ๐จ๐, probably due to a reduced absorption in the intestine. In addition, studies have shown that grapefruit juice has an impact on drug metabolism if taken ๐๐ฎ๐ซ๐ข๐ง๐  ๐ฆ๐ž๐๐ข๐œ๐š๐ญ๐ข๐จ๐ง ๐ข๐ง๐ ๐ž๐ฌ๐ญ๐ข๐จ๐ง or ๐ฐ๐ข๐ญ๐ก๐ข๐ง ๐Ÿ’ ๐ก๐จ๐ฎ๐ซ๐ฌ ๐›๐ž๐Ÿ๐จ๐ซ๐ž ๐ญ๐ซ๐ž๐š๐ญ๐ฆ๐ž๐ง๐ญ.
